Hyperhidrosis is excessive sweating. Sweating is necessary to keep the body cool. We already know that our sweat glands cause normal sweating, which should not really be a problem when controlled.

However, it should not interfere with a person’s ability to interact and function. Hyperhidrosis affects the quality of life. It may cause embarrassment and frustration. It can impact one’s ability to carry out routine chores and maybe an occupational hazard.

There are three types of hyperhidrosis: primary focal, generalized idiopathic, and secondary generalized.

Primary focal hyperhidrosis

This is a true condition not caused by medication or another medical problem. It occurs on specific areas of the body (focal), usually appearing on both sides equally. The most commonly affected areas are the feet, hands, underarms, head, and face. Primary focal hyperhidrosis frequently begins in childhood or adolescence, often starting with excessive sweating on the hands and feet that occurs at least once a week while the person is awake. There may be a hereditary component.

Generalized idiopathic hyperhidrosis

This is a true medical condition in which large areas of the body sweat. This is usually treated with oral medication.

Secondary generalized hyperhidrosis

This may be caused by a medical condition, such as menopause, an overactive thyroid, diabetes (peripheral neuropathy or stroke. Medications (antihypertensives or antidepressants), exercise, and heat also cause secondary generalized hyperhidrosis. Unlike primary hyperhidrosis, sweating involves large areas of the body and may occur during sleep. This condition must be investigated by a dermatologist for proper diagnosis and treatment.

Primary hyperhidrosis may affect a person’s quality of life, interfering with work and social activities. It causes emotional embarrassment and isolates people. It is psychologically unsettling and may be associated with depression and anxiety. Also physically debilitating, it may cause discomfort and skin irritation, which can lead to bacterial and fungal infections.

Hyperhidrosis treatment

There are many approved medications used to treat hyperhidrosis excessive sweating.

  • Prescription antiperspirant. Your doctor may prescribe an antiperspirant with aluminum chloride (Drysol, Xerac Ac). This product can cause skin and eye irritation. It’s usually applied to the affected skin before you go to bed. Then you wash the product off when you get up, taking care to not get any in your eyes. If your skin becomes irritated, hydrocortisone cream might help. Available by prescription and non-prescription, these topical sprays, gels, roll-ons, and lotions decrease sweating. The most common ingredient is aluminum chloride hexahydrate. Antiperspirants should be applied at bedtime on dry skin, covering the hair-bearing areas of the underarm (axillae) and washed off in the morning. Occlusion (covering the area) with plastic wrap may be necessary.
  • Prescription creams. A prescription cream that contains glycopyrrolate may help hyperhidrosis that affects the face and head.
    Nerve-blocking medications. Some oral medications block the chemicals that permit certain nerves to communicate with each other. This can reduce sweating in some people. Possible side effects include dry mouth, blurred vision, and bladder problems.
  • Antidepressants. Some medications used for depression can also decrease sweating. In addition, they could also help decrease the anxiety that worsens the hyperhidrosis.
  • Iontophoresis. Used to treat sweaty hands and feet, this treatment requires the patient to immerse the hands or feet in a shallow pan filled with water. A medical device sends a low voltage current through the water. This process is used every other day for about six to ten treatments until sweating decreases. Repeat treatments are necessary to maintain results. The patient administers the treatments at home. A prescription is necessary for the machine.
  • Melbourne Anti-wrinkle Injections like Botulinum toxin type A. A dilute concentration of this medication is injected by the dermatologist into the axillae (underarms), palms, or soles to decrease sweating. Botulinum toxin blocks the release of a neurotransmitter (acetylcholine) and is an effective treatment that may last four to eight months. Re-treatment is necessary to control excessive sweat.

Microwave therapy

With this therapy, a device that delivers microwave energy is used to destroy sweat glands. This involves two 20- to 30-minute sessions, three months apart. Possible side effects are a change in skin sensation and some discomfort. This therapy may be expensive and not widely available.

Sweat gland removal

If excessive sweating occurs just in your armpits, removing the sweat glands there may help. A minimally invasive technique called suction curettage may be an option, too, if you aren’t responding to other treatments.

Nerve surgery (sympathectomy)

This surgical procedure interrupts the autonomic nervous system impulses to the sweat glands. A potential side effect is compensatory sweating, excessive sweating that may be even more debilitating than the hyperhidrosis. Because of this side effect, which calls occur in up to 80% of patients, sympathectomy should be considered only for those patients who fully understand the risk and in whom other treatments have been properly tried and have failed. During this procedure, the surgeon cuts burns or clamps the spinal nerves that control sweating in your hands. In some cases, this procedure triggers excessive sweating in other areas of your body (compensatory sweating). Surgery is generally not an option for isolated head and neck sweating. A variation on this procedure interrupts the nerve signals without removing the sympathetic nerve (sympathotomy).

While you’re trying out different antiperspirants, or whatever other treatment your doctor recommends, you can also incorporate some of these at-home solutions to help reduce excessive sweating. What’s better about natural remedies is they do not have side effects.

Don’t wear heavy clothes that will trap sweat. Instead, wear light, breathable fabrics such as cotton and silk. Bring along an extra shirt when you know you’ll be exercising or outdoors in the heat. Your feet can sweat too, so wear socks that wick moisture away from them (merino wool and polypro are good choices).

Shower or bathe every day using an antibacterial soap to control the bacteria that can inhabit your sweaty skin and cause odors. Dry yourself completely afterward, and before applying antiperspirant.

Use underarm liners and shoe inserts to absorb sweat so it doesn’t ruin your clothes or start to smell.

Don’t order a double jalapeno burrito with a margarita at your favorite Mexican restaurant. Spicy foods and alcohol can both make you sweat, as can hot drinks like tea and coffee.

Excessive sweating is triggered by various factors and can vary from person to person. It may be normal for some people, but once sweating starts to affect your routine and becomes a nuisance on how you interact with people, you should look for solutions.
