Under Eye Bag Removal
There are several types of non-surgical eye treatments including dermal filler, PRP, and blepharoplasty
Dermal fillers are gel-like substances used to fill up the trough or area beneath the lower eyelid. Typically, they are made of hyaluronic acid – a sugar derivative that is naturally found in the body and can hold 1000 times its own weight in water. However, HA in dermal fillers is made in the laboratory. Its levels in our skin do change over time and our body modifies or breaks it down with enzymes as part of our metabolic processes. Dermal fillers have been used to successfully correct tear troughs. It is important to note though that tear troughs can be difficult to treat with dermal fillers. Overfilling the under-eye with dermal fillers can increase the appearance of tiredness.
PRP or Platelet Rich Plasma is a component of your blood that contains growth factors that serves various purposes, such as collagen production, for younger looking appearance. For bag removal, It is injected in the eye area that you want to have cosmetically done.
One of the most common non-surgical eye treatments is non-surgical blepharoplasty also known as eyelid reduction or eye bag removal. The natural process of aging and excessive sun exposure causes the skin to lose elasticity and blepharoplasty removes the excess skin and the fatty tissue surrounding the eye area. The treatment may be done using plasma resurfacing and may take longer than the usual injectable procedures. You may feel moderately uncomfortable and sensitive during your eyelid procedure since this is the area where the skin is at its thinnest.
Plasma resurfacing is a non-laser treatment that uses a plasma pen. Plasma energy comes out of a pen-like device.
During eyelid surgery, you will be given local anaesthesia, sedation, or general anaesthesia depending on the assessment of your doctor.
One advantage of dermal fillers is it lasts long but not permanently. The thick filler may last up to 12 to 24 months which will depend on the brand as each has its own cross linking signature.
To have a long lasting result, a top-up treatment is recommended after 6 to 12 months from the initial procedure.
It is comparatively safe but same with other procedures, there may be risks involved. The most common is discomfort or pain, bruising, and swelling. These are often minimal and sometimes avoidable. At times, some fillers create lumps and bumps under the skin if dealt with right away, usually within a week from placement, they can be manipulated by an injector and will quickly disappear. A more serious risk is vascular occlusion. It is a medical term for a blocked vessel. The person who will perform the procedure should understand your facial anatomy to know the best technique to avoid this and avoid placing too much filler in any one area in one sitting.
Blepharoplasty can also be surgical. It is an eyelid surgery that improves the shape of the eyelids and reduces the sagging of the skin. There are two types of blepharoplasty, the upper blepharoplasty which is the eyelid surgery that removes or reposition changing amounts of skin, muscles, and fats if there is already an existing hollowness in the upper eyelid while lower blepharoplasty is the eyelid surgery for the lower lid which includes eye bag removal.
A lower eyelid blepharoplasty is a more complex procedure that requires an incision under the eyelash line, running beyond the corner of the eye. Excess skin, muscles, and fats are removed or repositioned through this incision. At times, especially for younger patients, the main aesthetic issue is the underlying fat bulge. In this scenario, a transconjunctival “scarless” method through the inside of the eyelid is opted. In some cases, eliminating the fat/skin under the eyes can cause the eyes to appear more hollow and worsen the overall appearance. Hence, the need for further treatment to put the volume back into the area in the form of dermal fillers. Medications and ointments are prescribed to help in faster healing and to prevent infection after the procedure. You must wear sunglasses to protect the eye area until healing is completed.
Another option is a fat transfer from another part of the body. This involves minor liposuction to harvest fat from an area such as the abdomen and then injecting this fat into the under eye.
Bags under the eye happens when weakened and sagging. Excess skin relaxes which forms a pouch and the fat pads located under the eye slip down to fill space. Excess fluid in the body can also add up, making the lower lids even puffier and more swollen.

Fasting is required; meaning no food, water, cigarette, or gum because local anaesthesia with sedation or general anaesthesia will be required. It is also imperative not to wear any make-up or apply cream on the day of surgery. Patients should arrange their transportation going home because they won’t be allowed to drive themselves.
Before surgery, patients will be asked about their medical history including prescriptions, allergies, smoking, and previous surgery, etc. Patients are not allowed to take aspirin or other non-steroidal anti-inflammatory drugs, such as Ibuprofen or Voltaren 2 weeks before the surgery.
If medication is required for medical purposes like treating a heart condition, a Cardiologist will be consulted.
Smoking is also prohibited for a given period.
Upon waking up from surgery, you will have blurry vision. This is caused by a very thick antibiotic ointment that is applied in order to prevent the surface from drying. You will be asked to apply the same ointment 4 times daily inside the eye and rubbed into the wounds. Artificial tear drops can be prescribed to avoid dryness during the postoperative period. You will likely have moderate swelling and bruising of the skin which will subside 1 to 2 weeks for upper eyelid blepharoplasty and 2 to 4 weeks for upper and lower eyelid blepharoplasty. The degree will be different for each patient. Please keep in mind that postoperative swelling may take longer for few patients.
After a week from surgery, it is wise to use different measures to lessen the swelling. Ice packs should be applied over the treated area for 1 week. If specialised ice pack is not available, you may wrap a bag of frozen peas in a towel and apply it 3 to 4 times daily or more if needed.
You will be encouraged to sleep in a sitting position with several pillows under your bed to prevent associated swelling and fluid accumulation. It is also important that you refrain from strenuous activities for the 1st week from the day of surgery. Walking at a gentle pace is also recommended to ensure that they are mobile.
Blepharoplasty is rarely associated with significant pain. Appropriate pain killers will be supplied to you for cases that you will need them.
Recovery for lower lid blepharoplasty takes longer and patients may return to work 10 to 14 days after. However, you should be fine working from home using a computer 2 days after surgery, and in No way will it harm the eye or compromise the outcome.
